The periodic appointment is the ideal time to review your list of medications and renew them if necessary. Please bring a complete and recent list of all your medications, prepared by your pharmacy.
Your doctor will inform you of the next desired appointment for monitoring your health condition with a periodic examination.
It is your responsibility to note this date and call 2 or 3 weeks in advance to schedule an appointment.
In order to optimize access to healthcare, the periodic appointment replaces the annual appointment, as healthy adults do not necessarily need to meet with their family doctor every year. The suggested frequency of follow-ups is:
- Adults without any specific health issues under the age of 50 – Appointment every 2-3 years
- Healthy adults over the age of 50 – Appointment every 2 years
- Patients with chronic diseases such as: diabetes, hypertension, heart disease, or lung disease – Appointment every 3 to 6 months, depending on their health condition.

If you have a form to be filled out, please give it to your doctor during your visit or to the reception of the clinic. Many forms require a prior medical evaluation. It is often impossible to complete the documents on the same day.
According to the regulations of the Collège des Médecins du Québec, the doctor has thirty days to complete your form. However, the delay is often shorter.
These forms may include: life insurance, disability insurance, employer, CSST, SAAQ, medical exam summary, attendance certificate, etc.
Most of these forms have associated fees. If there are any fees to be paid, they will be indicated in this fee scheduleyou will be advised.
This amount constitutes the doctor's fees for administrative work not covered by the Régie de l'assurance maladie (RAMQ). A receipt will be issued for each form.
The receptionist will inform you when your form has been completed.

For patients without a family doctor, you must register with the Family Doctor Access Portal (GAMF). http//www.gamf.gouv.qc.ca/ to be referred to a family doctor in your area.

For any type of appointment with a doctor (mini-emergency, follow-up or periodic check-up), it's important that you respect the time slot of your appointment. A place is reserved for you!
Out of respect for the staff and professionals at our clinic who prepare your appointment, and especially out of respect for other patients waiting to see their doctor, you must cancel your appointment within 24 hours if you are unable to attend.
With this in mind, GMF Mercier will apply its no-show policy.
Missed appointments or cancellations with less than 24 hours' notice will be charged! *
Less than 24 hours:
- 30 fee for a consultation (follow-up, mini-emergency consultation)
- 50 fee for an initial appointment (1st appointment) or for a periodic examination

For some years now, the clinic's doctors have been receiving many requests for prescriptions without seeing the doctor, either by verbal request from the patient or by fax from the pharmacy. This practice is not to be encouraged and we try to avoid it as much as possible.. This wastes a lot of time and prevents your doctor and his team from treating you properly for your medical needs.
The renewal of your medication should be scheduled during your medical check-up with your family doctor.
You'll need to check with your pharmacist when your prescription expires, so that you can make an appointment with your family doctor within 1-2 months. It's up to you to plan your visit carefully, and to arrange for prescription renewals with your doctor.
Note that pharmacists can often renew or extend most of your prescriptions.
Certain exceptions may be prescribed on a walk-in basis, at your doctor's discretion and if your situation applies.

What is family physician management?
This is the longitudinal follow-up of a patient by a family doctor. During the intake visit, the family doctor familiarizes himself with the patient's state of health and history. He or she will then follow the patient over the years, carry out the periodic screenings required by his or her condition, and address any health problems that may arise.
Whocan be treated by a family doctor?
Family doctors see a wide variety of patients, from newborns to the elderly! All patients benefit from having a family doctor.
What if I don't have a family doctor?

The health insurance card allows you to benefit from services covered by the Quebec Health Insurance Plan. Make sure you always have a valid card by renewing it on time.
- Your RAMQ card must be valid and must be presented at each visit.
- Your RAMQ card is an identification tool and confirmation of your registration with RAMQ.
- If your card has expired or been lost, you can obtain an attestation of coverage from RAMQ by calling them directly. This must be done before your visit. At your visit, if the card has expired or you are unable to present it to us, you must pay the full cost of the visit and then apply to RAMQ for a full refund.

At GMF Mercier, Nurse Clinicians work closely with family physicians to manage patients with chronic diseases. This teaching and monitoring service is reserved for patients registered with a family doctor at GMF Mercier.
The patients under our care suffer mainly from :
- Diabetes
- Hypertension
- Dyslipidemia (cholesterol)
- Cognitive disorders (dementia)
- Chronic obstructive pulmonary disease
To be followed by the Nurse Clinicians, you must be registered with a family doctor at GMF Mercier.

Family doctors at GMF Mercier operate on an appointment-based system.You can make an appointment with your doctor at any time for a follow-up, an urgent visit or your regular check-up, if necessary.
When you need an appointment with your doctor, just call and you'll get an appointment within a reasonable time, depending on the doctor's availability. For urgent medical needs, you'll have the option of seeing another doctor on the GMF Mercier medical team with adapted access if your family doctor is unavailable.

Between birth and the age of 18 months, your child will see his or her doctor several times:
- 1 month
- 2 months
- 4 months
- 6 months
- 9 months (optional)
- 12 months
- 18 months and then yearly
- Annually up to 5 years
At each visit, your doctor will weigh and measure your child and check his or her motor and cognitive development.
Don't forget to bring your child's vaccination record with you on every visit.
